Athletic Physicals
The Ohio High School Athletic Association requires all student-athletes to have a valid physical completed before participating in athletics.  OHSAA rules allow any sports physical completed after May 1st to count toward the entirety of the following school year. Any physical completed afterward is only acceptable for one year from the date of the examination. Valid physicals are necessary for participation in all athletic activities including summer events and camps. Physicals can be performed at most medical facilities.  Community Memorial Hospital of Hicksville and CMH affiliates will charge the low rate of $30 when mentioning the physical is for a student-athlete.  Please use the physical tab on this page or pick up a packet at the school. These are the only two locations to get the complete physical packet that is accepted at the school.
